BOOKING FORMS
JMF Coaching is delighted to announce that you can now print off and fill in your booking forms at home! The booking forms will be updated regulary, meaning that you are up to date with our latest courses!
To use this option simply click on the name of the school or holiday course that your child would like to attend and open the document. You may then either fill in the form and email it back to us at jmfcoaching@yahoo.co.uk as an attachment, or simply print and return directly to our address or via the school office.
Please note: If your child is new to our courses and we do not already their information, you will be required to fill in a medical form on the first day of our courses. If you are unsure whether we hold your childs information, please ask a member of staff.
